Long Term Care ConsultationClay County Social Services is responsible for assessments of persons' overall care needs in order to determine whether community care or nursing home care is appropriate. The assessment includes notifying clients of cost-effective services available and the development of an individual service plan for those remaining in the community. Relocation Service Coordination Clay County Social Services is responsible for providing an assessment and transition assistance to persons under the age of 65 years who wish to move from a nursing facility to the community.
