Death Records

Clay County has access to death records of any person who died anywhere in Minnesota back to 1997 or any person who died in Clay County back to the 1872.  For death records outside of Clay County before 1997, contact the county registrar where the person died. 

Clay County Death Record Index On-Line  (1872 – 1980)

The Minnesota Department of Health maintains death records back to 1908.  You can search death records for 1908-1996 on-line at the Minnesota Historical Society.

Death Certificate Request

You can request a death certificate:  in person, by mail or by fax.

In Person (cash, check or credit card is acceptable)
1.    Complete a Death Certificate Application form indicating you have "tangible interest".  You can print and complete a form prior to coming to the Recorder's Office if you prefer.  Do not sign until a Deputy Registrar/Recorder can witness your signature.
2.    Show identification.
3.    Pay the corresponding fees by cash, check or credit card.
4.    Death Certificate Application Form (PDF:  197 KB / 3 pages).

 

By Mail (cashier's check, money order, or credit card)
1.    Complete a Death Certificate Application form indicating you have "tangible interest".  You can print and complete a form or you can call or e-mail the Clay County Recorder's Office to request that a form be mailed or faxed to you.
2.    Show identification and sign the completed application in front of a notary.  Have the notary sign the application and apply his/her notary seal.
3.    Make a photocopy of your identification.
4.    Mail the completed, notarized application and photocopy of identification, along with a cashier's check, money order or credit card information. 

Convenience fee for a credit card transaction:
     -- $65.00 or less - $2.00

     -- Over $65.00 - 3%

The following credit cards are accepted:

     VISA®

By Fax (credit card only)
