Birth Records
Clay County can make
birth certificates for most people born in Minnesota. For birth
records prior to 1935 Clay County can assist you but it
may take an extra day.

ordering a birth certificate from another state.

Certificate Request
You can request a birth certificate:
in person, by mail or by
fax.

1. |
Complete a Birth Certificate Application form indicating you have
"tangible
interest". You can print and complete a form
prior to coming to the Recorder's Office if you prefer.
Do not sign
until a Deputy Registrar/Recorder can witness your signature. |

2. |
Show identification. |
|
3. |
Pay the corresponding fees
by cash, check or credit card. |
|

Show identification and sign the completed application in
front of a notary. Have the notary sign the application and
apply his/her notary seal. |

Make a photocopy of your identification. |

4. |
Mail the completed, notarized application and photocopy of
identification, along with a cashier's check, money order or credit
card information.
A convenience fee will be charged for credit card
transactions.
