Birth Records

Clay County can make birth certificates for most people born in Minnesota.  For birth records prior to 1935 Clay County can assist you but it may take an extra day.

Some records are confidential and can only be obtained by some people.

If you were not born in Minnesota, see the National Center for Health Statistics for more information about ordering a birth certificate from another state.

Birth Certificate Request

You can request a birth certificate:  in person, by mail or by fax.

In Person (cash,  check or credit card is acceptable)
1.    Complete a Birth Certificate Application form indicating you have "tangible interest".  You can print and complete a form prior to coming to the Recorder's Office if you prefer.  Do not sign until a Deputy Registrar/Recorder can witness your signature.
2.    Show identification.
3.    Pay the corresponding fees by cash, check or credit card.
4.    Birth Certificate Application Form (PDF:  410 KB / 2 pages)

 

By Mail (cashier's check, money order, or credit card)
1.    Complete a Birth Certificate Application form indicating you have "tangible interest".  You can print and complete a form or you can call or e-mail the Clay County Recorder's Office to request that a form be mailed or faxed to you.
2.    Show identification and sign the completed application in front of a notary.  Have the notary sign the application and apply his/her notary seal.
3.    Make a photocopy of your identification.
4.    Mail the completed, notarized application and photocopy of identification, along with a cashier's check, money order or credit card information. 

A convenience fee will be charged for credit card transactions.

5.    Birth Certificate Application Form (PDF:  410 KB / 2 pages)

 

By Fax (credit card only)
1.    Complete a Birth Certificate Application form indicating you have "tangible interest".  You can print and complete a form or you can call or e-mail the Clay County Recorder's Office to request that a form be mailed or faxed to you.
2.    Show identification and sign the completed application in front of a notary.  Have the notary sign the application and apply his/her notary seal.
3.    Make a photocopy of your identification.
   4.    Fax the completed, notarized application and photocopy of identification, along with your credit card information.
